(option 1) within 31 days of your hire date to waive
coverage. If you don’t want to be enrolled in the 401(k)
Savings Plan, you must contact the Chevron Phillips
Pension and Savings Service Center at 1-866-771-5225
to waive enrollment.

3. Review your confirmation statement to ensure your enrollment elections were recorded
Your confirmation statement will be mailed to you within 7 – 10 days after you enroll.
Aetna will send you plan ID cards for you and your dependents.
If you signed up to participate in a Health Care Flexible Spending Account (HCFSA), you will be sent
a PayFlex FSA Debit Card.
If you enrolled in the
Value CDH Plan
, you must set up your Health Savings Account (HSA) with
Fidelity (see page 34 for information on how to set up your HSA). Fidelity will send you an HSA Debit
Card to pay health care providers directly from your HSA.
